Maj. Gen. S.P. Kapoor, Member—This is an appeal against the order dated 3.5.2004 of District Consumer Disputes Redressal Forum-I. U.T., Chandigarh [hereinafter for short referred to as District Forum-I] in Complaint Case No. 502 of 2003, Rita Rani v. LIC & Ors.
2. Briefly the contention of the complainant is that she is holder of LIC policy - Asha Deep Plan bearing No. 171142978 for a sum assured of Rs. 50,000/- for a period of 10 years. This policy was taken by her on 15.8.1996. It is contended that on 6.3.2000 the complainant started showing signs of breathlessness and was taken to G.B. Pant Hospital, New Delhi (O.P. No. 4) where it was detected that Mitral valve of the complainant’s heart needed replacement. The complainant was admitted with O.P. No. 4 on 10.10.2001 and the operation was conducted on 23.10.2001. The complainant was discharged from the hospital on 1.11.2001 and she spent approximately Rs. 1.5 lacs on the said treatment. Under the LIC Policy - Asha Deep Plan she filled her claim form along with the report of the Cardiologist and the discharged summary issued by O.P. No. 4 and submitted the same to the O.Ps. for payment. However, vide its letter dated 17.8.2002
