JUDGMENT

S.C. Mohapatra, President• Deficiency in insurance service is grievance of complainant.

2. Husband of the complainant insured his life with Opposite Party No.1 under two policies which commenced from 23.2.1989 and 28.3.1990. Both policies are under double acci• dent benefits scheme. Complainant is nominee under both the policies. On 26.11.1990 husband of complairiant sustained fatal injuries in a road accident and died while being treated in the hospital on the same day. Complainant intimated the fact of death to Opposite Party No. 1 in her letter dated 28.12.1990. Thereafter, claim forms A, B. B-1 and C were sent to her by Opposite Parties and they were submitted back after being duly filled in. Since death of insured was within two years from the date of commencement of the risk, it was treated to be an early claim for which inquiry was necessary to b~ made by Opposite Parties to find out the bonafides of the claim and to rule out any fraud in larger interest of the policy holders as well as of the corporation. On receipt - of the claim forms, complainant was requested in letter dated 9.3.1991 to return the discharge certificate after due execution of the same along with c
