ORDER
K.S. Chaudhari, (P.M) - Both these revision petitions arise out of single order of State Commission, hence, decided by common order.
2. R.P. No. 3881 of 2013 has been filed by United India Insurance Co. Ltd./ opposite party and R.P. No. 3341 of 2014 has been filed by Mili Dutta/complainant against order dated 8.7.2013 passed by Learned State Commission in FA No. 480/2012- United India Insurance Co. Ltd. & Anr. v. Smt. Mili Dutta & Anr.; by which while allowing appeal, order of District Forum allowing complaint was modified.
3. Brief facts of the case are that complainant-Mili Dutta took Mediclaim Policy from the OP for the period from 04.06.2008 to 03.06.2009 under the Health Insurance Gold Policy conditions for a sum insured of Rs. 3,00,000/- with cashless benefit of treatment and renewed the policy every year, and lastly for the period from 04.06.2010 to 03.06.2011 vide policy No. 03700/48/10/97/00000065, Identity Card No. HHS 4.0700321871 issued by the OP for cashless treatment.
