1. The Appellant filed an RTI application dated 30.10.2023 (offline) seeking the following information:

“I am pooja Omprakash verma Sister of Bipin omprakash Verma. my father. Mr. Omprakash verma is a central railway employee. My brother Bipin Omprakash verma is admitted in pune divisional hospital in month of August from 1st August 2016 to 8th of August 2016 and treatment is going on for around 1 month from, divisional hospital itself. on 31st of August 2016 his food pipe is blocked, then we admit him in pune divisional hospital.

In this serious situation divisional hospital keep the patient for 24 hrs, then referred the patient in budharani hospital pune.

I want some Information regarding the above mention period and situation.

1). We want all the reports, Scan, test and all other documents of my brother...

2). What disease treatment doing by divisional doctor during this above mention period and what is the Symptoms of patient?

3). What medicine is prescribe by divisional doctor + and what is the impact of that medicine on patient body.

4). In this serious situation, after blocking the food pipe on which basis divisional hospital keep the patient for around 24 hours.

5). What treatment, medicine they provide in this 24 hours, after blocking the food pipe.

6). on which basis railway refer the case budhrani hospital (tie-up hospital) after keeping the patient for around 24 hours., to As above mentioned railway hospital refer the case to budhorani hospital and that hospital give further treatment to the patient from 2nd September to around 1st of oct 2016. I want some information regarding treatment of my brother in above mentioned period.

1). We want all the reports, scan, Test documents. of my brother.

2).Which disease he suffered from?

3). What treatment, medicine prescribed by doctor?

4). How much fund allotted by railway to budharani hospital for treatment of nay brother and provide distribution of that fund.

5). on which basis budharani hospital discharged my brother, when is not well in that time?

When budharani discharged the patient around .....and of oct 2016. We, admitted him in divisional hospital, again.

keep the patient for around. 15 days and. the infection is spread in his full body Infection in his body is soo much that he facing difficulty in breathing also.

I want information regarding this around. 15 days treatment of my brother.

1)In this serious situation of the patient, on which basis divisional hospital keep the patient for around 15 days.

2) In this serious situation of patient why railway's doctor not refer the case to another tie-up hospital?

3). What treatment, medicine is provide by doctors during this 15 days?

4) we follow everything like prescription, medicine guidelines of railway doctors and I lost my brother. So who take responsibility for his death.

5). If deficiency in service performed by doctor. What remedies are available to my family?”
