1. Briefly stated relevant facts of the case are that the Anurada @ Shobha/ the petitioner/the complainant/ (hereinafter referred to as “the petitioner”) was wife of Siddlingeshwara @ Siddanna (hereinafter referred to as “the insured”) who during his life time had taken two life insurance policies bearing no. 665276493 for assured sum of Rs. 5,00,000/- and bearing no. 665283885 for assured sum of Rs. 65,000/- from the respondents/the opposite parties/Life Insurance Corporation (hereinafter referred to ‘the respondents’). The insured nominated the petitioner as nominee and paid the premium till his death. The insured died on 14.06.2011 in a road accident. The petitioner being nominee submitted claim form to the respondent no 1 along with requisite documents such as death certificate, two original policies, FIR and complaint of accident etc. The respondent no 1 forwarded death claim papers in respect of two policies to the respondent no. 2 for settlement of claim.
